This is definitely not looking good for Mr. Helms. 
  • Terrible chest film. Suggests neoplasm.
  • Bad chemistry labs. What do you think they suggest?
  • Lousy history. Smoking and father dying of lung cancer.
The lung mass needs to be evaluated pronto.
  • A sputum for cytology is quick, easy and cheap. We can see the cells plus get a culture.
  • Skin test for TB or Histoplasmosis might help, but if it is one or the other, he is likely to have such a profound cutaneous response he may actually ulcerate and lose some skin. Tread carefully here
  • Bronchoscopy certainly seems indicated, but can you get him talked into it?
