Mr. Helms appears older than his stated age of 47 years. 
  • Respirations 18/min and mildly labored.
  • Temp = 99 F
  • Pulse = 88 and regular.
  • BP = 138/86 mmHg 
  • Weight = 174 pds (a loss of 16 lbs he believes)
Physical findings.
  • Throat slightly reddened. No exudate.
  • Questionable ptosis of right eyelid.
  • Heart sounds are normal, no murmurs or third heart sounds.
  • Chest reveals mild expiratory wheezes and a few ronchi.
  • Dullness to percussion and reduced breath sounds over the apex of the left lung.
  • No abdominal masses and bowel sounds are WNL.
  • Pedal pulses are diminished bilaterally.
  • There appears to some enlargement of the tips of his fingers.
