Physical exam reveals a well nourished adult male in moderate distress owing to continued abdominal pain and nausea.
  • Blood pressure supine = 128/88 mmHg
  • Blood pressure standing = 100/68 mmHg
  • Pulse = 106 and regular, respirations = 14 per min.
  • Temperature = 98.6 F
  • Abdominal exam reveals mild tenderness in right upper quadrant, good bowel sounds and no masses.
  • Lungs are clear and heart sounds are normal
  • No adenopathy and peripheral pulses are good.
  • Mucous membranes and nail beds are pale.
  • Eye grounds are unremarkable and pupils are equally reactive to light and accommodation (PERLA) 
  • No masses noted on rectal exam, small amount of black stool is present.
  • The stool test positive for occult blood.
