Mr. TE's leg infection was due to a beta hemolytic streptococcus. The red streaks up his leg, a lymphangitis, gives it away. Although he is being treated, he has already begun making the antigen/antibody complexes that are sticking in his renal glomeruli. Complement is activated, chewing holes in the filtering membrane. The red blood cell casts indicate there is an acute glomerulonephritis. The RBC casts are leaking through the substantially damaged glomerular basement membrane. What about casts again?
How about a look at his renal biopsy? Steroids might help, but it's hard to say what the outcome will be. He might progress to rapidly progressive glomerulonephritis (RPGN).
