Mr. Jones' past history seems non-contributory on the surface.
  • exercises moderately
  • smoked approximately 1-2 packs of cigarettes a day for 15 years, but quit 10 years ago
  • describes himself as a social drinker
  • reports no particular food intolerance
  • does not drink coffee
  • Liberally uses aspirin for arthritic knee and hip pain.
His most recent problem has been epigastric pain of about one month duration, but things changed dramatically last night.
  • he was awakened during the night with severe abdominal pain, but unlike previous episodes,
  • he vomited blood for practically an hour, and
  • he threw up what he describes as "coffee ground" looking material
