Sometimes the only spike is in the urine.   Serum and urine electrophoresis on the same patient. In the case of a pure light chain myeloma, the M protein clears in the urine. There is almost none seen in the serum. The serum protein electrophoresis even shows a low gammaglobulin. The urine protein electrophoresis has the concentration and spike.
