Histologic
Grading of Prostate Carcinoma
The
Gleason grading system is most often used.

This is
a histologic assessment of the tumor, not a description of its spread.
-
Five
grades based on glandular pattern and degree of differentiation.
-
Grade
1 is best differentiated with well developed glandular pattern.
-
Grade
5 is least differentiated with no glandular pattern.
-
The other
grades are in between.
-
A tumor
is graded on both its best and worst areas of differentiation.
-
The two
number values are added to give the Gleason's Score.
-
For example,
a tumor with patterns of both Gleason's 2 and 4 would have a Gleason's
Score of 6.
