So
what does the bug do to cause injury?
-
H. pylori elaborates
a platelet activating factor that causes thrombotic occlusion of the superficial
capillaries of the mucosa, thereby leading to ischemic injury.
-
Colonization of the mucosa leads
to chronic inflammation which enhances susceptibility to acid injury.
-
The damaged mucosa leaks micronutrients
onto its surface, thereby creating a local environment favorable to H.
pylori.
-
The free ammonia elaborated
by the bacteria is thought to stimulate the release of gastrin, leading
excessive acid production (a feature considered of importance in the formation
of duodenal ulcers).
Other factors associated with
ulcer formation.
-
Non-steroidal anti-inflammatory
drugs suppress the prostaglandin synthesis, thereby interfering with healing.
