Summary of history and physical
  • No significant previous illnesses or injuries.
  • Non-smoker and doesn't drink alcohol.
  • No menstrual irregularities.
  • ROS is unremarkable.
  • T = 98.6; R = 20/min and regular; P = 88/min and regular
  • PERLA
  • Chest is clear to auscultation. No dullness to percussion.
  • Heart sounds are normal, no murmurs, clicks or 3rd sounds.
  • Abdomen is soft with no organomegaly. Bowel sounds good.
  • Strong peripheral pulses.
  • Pelvic exam reveals slight enlargement of the left ovary.
  • No localizing neurological signs.
