When distant metastasis are present, cure is unlikely.
  • Long term remissions are possible, especially in hormonally responsive tumors.
  • Lymph node metastasis: most important prognostic factor.
    • No involvement = 70% to 80% 10-yr survival.
    • One to three nodes = 35% to 40% 10-yr survival
  • Locally advanced disease: Skin, fascia or muscle involvement are often associated with distant metastasis.
  • Tumor size: the smaller the lesion the better.
    • < 1 cm = 98% 5 yr survival
    • < 2 cm and no nodes = 96% 5-yr survival
  • Histology: the less well differentiated the tumor, the poorer the prognosis. Mitotic rate is very important in grading.
  • ERA/PRA status: cancers with high levels of hormone receptors have a slightly overall better prognosis than those without receptors.
