Time to narrow things down.
  • Rheumatic fever is possible, but unlikely.
    • The arthritis is not migratory.
    • History doesn't fit.
  • Osteoarthritis doesn't seem likely.
    • Typically osteoarthritis involves distal hand joints.
    • Age isn't consistent.
    • Weight bearing joints are spared in our patient.
  • Septic arthritis seems to be out.
    • Septic joints are always hot, painful and swollen.
    • Besides, CBC doesn't reflect an acute inflammatory episode.
  • Gout doesn't seem too likely.
    • Uric acid isn't up and these aren't the usual joints.
  • Psoriatic arthritis.
    • History and physical don't fit.
  • Arthritis associated with inflammatory bowel disease.
    • History and physical don't fit.
    • Wrong joints involved for this one.
