Pulse = 88/min and regular

B/P = 118/70 mmHg

Respirations = 20/min and without
labor
Physical
-
Mild scleral and skin icterus;
palmar erythema.

Mild gynecomastia, and
little red angiomas of the chest.

Heart sounds are normal. No
murmurs or third heart sounds.

A few crackling rales
in the chest fields bilaterally.

Right upper quadrant abdominal
tenderness. Liver is two finger breadths below the coastal margin.

Spleen is palpable.
-
Bowel sounds normal, questionable
fluid wave.

Rectal reveals normal prostate
size, stool positive for occult blood.
