There are basically two ways to go:
  • Oral replacement with ferrous sulfate:
    • 325 mg tid (three time a day), taken between meals.
    • Typically requires about 6 months of therapy.
    • Use of vitamin C with the iron will keep the iron in the reduced state and facilitate absorption.
    • About 25% of people will experience fairly annoying GI side effects:
      • nausea
      • cramping
      • diarrhea or even constipation.
      • Noncompliance is the primary reason for failure or delayed improvement with oral therapy.
  • Parenteral therapy IM or IV.
    • Useful  with malabsorption, inflammatory bowel disease, etc.
    • High requirement.
    • Intolerance to oral preparations.
