Initial physical findings:
  • Temperature = 99.4F
  • Pulse = 88/min and regular
  • B/P = 118/70 mmHg
  • Respirations = 20/min and without labor
Physical
  • Mild scleral and skin icterus; palmar erythema.
  • Mild gynecomastia, and little red angiomas of the chest.
  • Heart sounds are normal. No murmurs or third heart sounds.
  • A few crackling rales in the chest fields bilaterally.
  • Right upper quadrant abdominal tenderness. Liver is two finger breadths below the coastal margin.
  • Spleen is palpable.
  • Bowel sounds normal, questionable fluid wave.
  • Rectal reveals normal prostate size, stool positive for occult blood.
