It is definitely time to stop and think.
  • Middle aged man presents with confusion, abdominal pain, nausea and vomiting.
  • He's icteric, has gynecomastia and has little "spiders" on his chest.
  • Alcoholic brother.
  • Palpable spleen and possible abdominal fluid wave.
  • Stool positive for occult blood.
The story so far adds up to something being wrong with one or more of the following:
  • liver
  • pancreas
  • gastrointestinal tract
