• Open the form in something you can type directly to.
• Cut and paste the completed form into an email message.
• Be sure you're online, and then send the email.
• Microsoft Word users,
• You'll be asked if you want to open or save the form.
• Open it, complete the form, copy & paste to an email message.
• Click here for the Word document -> evaluation form.
• WordPerfect users, for you it's a little more complicated.
• Right click (right sided button on your mouse) on the link below.
• Choose 'Save Target as...", and save to your desk top.
• Open WordPerfect and then open the eval.wpd file.
• Complete the form, then copy and paste.
• Right click here for the WordPerfect file -> evaluation form.
• Type evaluation on the subject line of the email and send it to me.
