Endocrinopathies are the most common paraneoplastic syndrome.
Cushing's syndrome
- Paraneoplastic Cushing's
syndrome is one of the more common.
- Approximately 50%
with paraneoplastic Cushing's have it as a result of a small cell cancer
of the lung. Less commonly, pancreatic carcinomas and neural tumors.
- We see an elevation in
both tumor derived ACTH and propriomelanocortin (POMC).
- POMC is not elevated
in true pituitary Cushing's Disease.
Hypercalcemia
- Probably the
most common paraneoplastic endocrinopathy.
- By definition, it is
not due to skeletal metastasis.
- Parathyroid Hormone
Related Protein (PTHrP) from squamous cell carcinoma of lung, breast
cancer, renal cell carcinoma, ovarian carcinoma and even some lymphomas.
