Endocrinopathies are the most common paraneoplastic syndrome.
 
Cushing's syndrome
  • Paraneoplastic Cushing's syndrome is one of the more common.
  • Approximately 50% with paraneoplastic Cushing's have it as a result of a small cell cancer of the lung. Less commonly, pancreatic carcinomas and neural tumors.
  • We see an elevation in both tumor derived ACTH and propriomelanocortin (POMC).
  • POMC is not elevated in true pituitary Cushing's Disease.
Hypercalcemia
  • Probably the  most common paraneoplastic endocrinopathy.
  • By definition, it is not due to skeletal metastasis.
  • Parathyroid Hormone Related Protein (PTHrP) from squamous cell carcinoma of lung, breast cancer, renal cell carcinoma, ovarian carcinoma and even some lymphomas.
