No question about it, if you blink, you’ll miss
this little guy. This is a little endoscopic biopsy from someone who was being
worked up for malabsorption.

The mucosal atrophy in this section is pretty
profound. You will see no villial folds, and overall the mucosa is just a shadow
of what it should be. There is considerable inflammation in the lamina propria,
and not lymphoid aggregates as you would expect. Instead, it’s throughout, and
is part of the destructive process. See how many eosinophils you can find.
Remember, this is a condition of gluten sensitivity, but once the bowel gets to
this stage, there is sparingly little absorption going on. The bacteria of the
gut love it though. The hemorrhage seen here is an artifact of the biopsy
procedure.
