  • She has a febrile illness and is obviously jaundiced.
  • Her CBC is consistent with a viral illness.
    • Increased lymphocyte percentage and presence of reactive lymphs.
  • Her chemistry profile shows elevation of agents relative to the liver:
    • AST is 603 U/L (nl = 5-55)
    • Bilirubin is markedly up at 6.7mg/dL (nl < 1.5)
    • Both her GGTP and alkaline phosphatase are up, but not strikingly so.
 
  • If we had a biopsy of her liver, it would certainly show evidence of liver cell necrosis.
    • Apoptotic cell (arrow)
    • Inflammation and necrosis (circles)
