Acute lymphocblastic leukemia / lymphoma (ALL)
  • Approximately 2,500 new cases each year in the US.
  • Most are under 15 years of age, with peak for ALL of 4 years.
    • Does occur in adults but very infrequently.
  • Whites more common.
  • Pre-B cell acute lymphoblastic leukemia.
    • Pre-B cells express terminal deoxytransferase (TdT) activity.
    • Lack surface IgG
    • Positive for CD19; +/- for CD10 and CD20
    • Many chromosomal translocations invloving the Ig locus; t(12;21), t(9;22) the Philadelphia chromosome and t(4;11).
    • Children predominate, with symptoms of cytopenias, due to extensive bone marrow involvement.
  • Pre-T cell acute lymphoblastic lymphoma
    • Pre-T cells express TdT enzyme activity and CD1 antigen.
    • Variable expression of other T-cell markers, CD2, CD3, CD4....
    • Multiple chromosomal translocations, most invlove T-cell receptor loci.
    • Adolescent males predominate.
    • Mediastinal and thymic involvement.
    • Variable spleen, liver and marrow involvement.
