Acute
lymphocblastic leukemia / lymphoma (ALL)
-
Approximately 2,500 new cases
each year in the US.
-
Most are under 15 years
of age, with peak for ALL of 4 years.
-
Does occur in adults but very
infrequently.
-
Whites more common.
-
Pre-B cell acute lymphoblastic
leukemia.
-
Pre-B cells express terminal
deoxytransferase (TdT) activity.
-
Lack surface IgG
-
Positive for CD19; +/-
for CD10 and CD20
-
Many chromosomal translocations
invloving the Ig locus; t(12;21), t(9;22)
the Philadelphia chromosome and t(4;11).
-
Children predominate,
with symptoms of cytopenias, due to extensive bone marrow
involvement.
-
Pre-T cell acute lymphoblastic
lymphoma
-
Pre-T cells express TdT
enzyme activity and CD1 antigen.
-
Variable expression of other
T-cell markers, CD2, CD3, CD4....
-
Multiple chromosomal translocations,
most invlove T-cell receptor loci.
-
Adolescent males predominate.
-
Mediastinal and thymic
involvement.
-
Variable spleen, liver and marrow
involvement.
