Mr. TE has the nephritic syndrome, secondary to a strep infection.
  • Mr. TE's leg infection was due to a beta hemolytic streptococcus.
  • The red streaks up his leg, a lymphangitis, gives it away.
  • Although he is being treated, he has already begun making the antigen/antibody complexes that are sticking in his renal glomeruli.
  • Complement is activated, chewing holes in the filtering membrane.
  • The red blood cell casts indicate there is an acute glomerulonephritis.
  • The RBC casts are leaking through the substantially damaged glomerular basement membrane.
