Quiz Please

Mr. GS  has an impacted gallstone.
  • History and physical fits.
  • Large amount of bilirubin in the urine.
  • Absent urinary urobilinogen.
    • With obstruction, no bile gets to the gut.
    • Normally, there is bacterial breakdown of the bilirubin into urobilinogen.
    • Normally, urobilinogen (colorless) is reabsorbed.
    • Normally, urobilinogen is cleared in the urine.
    • Without a functional hepato-billiary-GI circuit, there can be no urobilinogen in the urine (think about it).
  • He needs help getting rid of this stone.
  • Bad outcomes could include sepsis, pancreatitis, on and on.
  • Want to see the stone?

Glad we got that figured out before anything bad happened. What's next?           Back
