  • History and physical consistent pyelonephritis.
  • Fever and flank pain.
  • High WBC count with left shift.
  • Protein, blood, nitrites, WBCs in the urine.
  • Presence of WBC and epithelial casts tell us the infection is in the kidney proper, and not just lower urinary tract.
