Broad or waxy casts
  • Look like a wax plug with cracked ends.
  • Made mostly of protein and dead cells.
  • Severe chronic renal disease.
  • Malignant hypertension.
Hyaline casts
  • Frequently seen, composed of renal secreted protein, Tamm-Horsfall protein.
  • No particular disease associated.
  • Often seen in healthy people with otherwise normal urine studies.
Granular casts
  • Degenerated cellular casts.
  • May represent congealed protein.
  • Often associated with significant renal disease, rarely seen following strenuous physical exercise.
