Remember this stuff about casts? It's important.
 
Red blood cell cast
  • Indicates renal hematuria
  • Always pathological
  • Glomerulonephritis, SBE, SLE, rarely CHF and renal vein thrombosis
White blood cell casts
  • Renal infection (interstitial tissue).
  • Acute pyelonephritis
  • Non-infectious inflammatory states: SLE
  • Rarely glomerular disease
Epithelial cell casts
  • Nephrotoxic agents: acetaminophen
  • Some viruses: CMV, hepatitis.
  • Severe glomerular injury which may lead to tubular cell death
