Fever patterns

  • Normal diurnal variation
    • Peak temperature in late afternoon or early evening
    • Variation will persist, even in patients with fever.
  • Rigor, a true shaking chill and fever
    • Bacterial infection
    • Sepsis
    • Less commonly viruses and drugs
  • Hectic fever (wide swings)
    • Abscess
    • Disseminated tuberculosis
    • Collagen vascular diseases

  • Relapsing fever, typical of malaria
    • Characterized by episodes of shaking chills and high fever, separated by 1-3 days of normal temperatures.
  • Lowered body temperature may be seen with:
    • Diabetic ketoacidosis, uremia and hepatic failure
