Morphology of the lesion

  • Macule: area of altered color without change in elevation or texture.
  • Papule: raised solid lesion < 5mm diameter.
  • Nodule: raised solid lesion > 5mm diameter.
  • Plaque: plateau-like solid elevation.
  • Vesicle: fluid filled lesion < 5mm diameter.
  • Bulla: fluid filled lesion > 5mm diameter.
  • Wheal: transient, localized area of edema and reddening.
  • Scale: thickened area of stratum corneum.
  • Crust: dried serum or pus
  • Ulcer: localized area of denuded skin
