Chronic myelogneous leukemia (CML)
- CML is a clonal stem
cell malignancy.
- Primarily adults, ages
25-60. Peak 30-40.
- Distinctive genetic abnormality,
t(9;22)(q34;q11).
- Translocation involves
BCR gene and The ABL gene.
- The resulting fusion
BCR-ABL gene produces a novel protein with tyrosine
kinase activity.
- This novel gene product
causes run away proliferation in the affected stem cell.
- The residual chromosomal
fragment of chromosome 22 is known as the Philadelphia Chromosome
(Ph')
- The malignant stem cell
carrying the t(9;22) translocation reproduces its own kind and
- Rapidly generates "diseased"
granulocytic precursors, leading numerous functionless PMNs.
- In time, the healthy
bone marrow constituents are replaced by the progeny of this sick stem cell.
