Myelodysplastic syndromes develop in two settings:
  • Idiopathic or primary MDS
    • Unknown etiology
    • develops insidiously
    • typical patient is over 50
  • Therapy related MDS (t-MDS)
    • Complication of chemotherapy (especially alkylating agents) and/or radiation therapy.
    • Usually within 8 years of exposure.
    • Highest rate of progressing to AML
Morphologic changes of bone marrow in MDS
  • Disordered (dysplastic) maturation of all three cell lines in bone marrow (erythroid, myeloid and megakaryocytic).
  • Dysplasia of erythroid line is most easily seen.
