Myelodysplastic
syndromes develop in two settings:
-
Idiopathic or primary MDS
-
Unknown etiology
-
develops insidiously
-
typical patient is over 50
-
Therapy related MDS (t-MDS)
-
Complication of chemotherapy
(especially alkylating agents) and/or radiation therapy.
-
Usually within 8 years of exposure.
-
Highest rate of progressing
to AML
Morphologic changes of bone marrow in MDS
-
Disordered (dysplastic) maturation
of all three cell lines in bone marrow (erythroid, myeloid and megakaryocytic).
-
Dysplasia of erythroid line
is most easily seen.
