Since the PT and aPTT overlap, you can exclude some proteins depending on which is prolonged.
  • If the PT is prolonged, but the aPTT is normal, there's only one possibility. Factor VII.
  • A prolonged aPTT with normal PT is a little more complex.
  • To the right is an algorithm that provides the basics.
  • First off, you know the defect is not in the final common path and it can't be factor VII.
  • So it has to be a protein(s) unique to the aPTT.

What if both aPTT and PT are long?

  • Multiple defects, or
  • Final common path defect.
  • Once narrowed down, get specific coagulation factor levels.
