How can there be clot formation in the face of a prolonged aPTT?
 
  • Lab error? Even her platelet count is down.
  • Physical certainly suggests leg vein thrombosis.
  • She may have early rheumatoid arthritis.
    • Her symptoms plus positive RA.
  • History of miscarriage
  • Family history may be important.
  • Definitely want to avoid what's on the right.
  • Repeat of the aPTT still shows substantial prolongation at 47 seconds.
  • Remember with anti-cardiolipin antibodies, the aPTT may be prolonged.
    • This prolongation is a fluke of the artificial way the aPTT is triggered.
  • Could this be antiphospholipid syndrome (APS)? Here are some clinical findings.
