Chronic myeloproliferative disorders
  • The multipotential stem cell is the neoplastic cell in these conditions.
  • The neoplastic stem cell does the following:
    • Clones itself, as all stem cells do.
    • Gives rise to one or more of the following:
      • a malignant erythroid line.
      • a malignant myeloid line
      • a malignant megakaryocytic line.
  • In time, these deranged cell lines replace the bone marrow, effectively squeezing out and suppressing the healthy population.
  • Chronic myeloproliferative disorders differ from other leukemias in that initially the malignant cell lines do complete differentiation to their respective "terminal" cell.
    • RBC
    • WBC and/or
    • Platelet forming megakaryocytes.
  • This unregulated proliferation of the neoplastic derived lines results in
    • Marrow hypercellularity with increased hematopoiesis, and
    • Elevated peripheral blood counts.
