Joseph's physical exam continued.

  • Chest is clear and there are no abnormal heart sounds.
  • The abdomen is soft without organomegaly. There is no obvious tenderness.
  • Extremities reveal no edema, although there are multiple fresh abrassions on the flexor surfaces of both legs.
  • Neurological exam:
    • Patient could only follow simple commands.
    • There was no facial droop.
    • Reflexes were equal on all extremities.
    • Muscle strength, sensory and detailed cranial nerve exam were unobtainable.
At the time of admission several labs and a skull series were ordered.
  • Skull X-rays revealed no evidence of skull fracture.
  • Hemogram was ordered.
  • An ELIZA for HIV is drawn.
  • LP was not done because Joseph was afebrile.
A cranial CT is ordered to rule out a subdural an LP is done to rule out meningitis.
