Steroids are indicated for treatment of TB under certain circumstances.

  • Tuberculous meningitis
  • Tuberculous pericarditis
  • Severe cachexia in pulmonary TB

Steroids and treatment of tuberculosis.

  • Rifampin increases metabolism of prednisone by 50%, so the dose needs to be 120 mg to get the effect of 60 mg.
Gladys' differential should include:
  • Viral pericarditis/myocarditis
  • Rheumatic heart disease
  • Fungal infection
  • Malgiancy
Diagnosis is not always straightforward.
  • Pericardial fluid is positive for AFB only 50% of time.
  • Pericardial biopsy has only slightly higher yield.
