Lobar Pneumonia

  • Classic pneumococcal lobar pneumonia remains common in Kenya and throughout Africa.
  • The situation is much like that of the pre-antibiotic era in the United States.
  • Typical patient
    • Previously healthy young adult.
    • The usual signs
      • Tachpnea,
      • Fever,
      • Rusty sputum
    • Not so common signs
      • Tender and enlarged liver (60%)
      • Jaundice (20%)
      • Myalgia
      • Diarrhea
  • Penicillin is still the first line drug, although resistant strains are known.
  • Other organisms associated with lobar pneumonia:
    • S. aureus
    • Klebsiella species
    • Mycoplasma pneumoniae
    • Mycobacterium tuberculosis
