A CD4 count is simply too expensive to order routinely.
The differential diagnosis for her pulmonary symptoms should include:
- Miliary tuberculosis
- Malignancy with lymphangitic spread.
- Pulmonary embolus
- Atypical pneumonia should be kept in mind, but generally doesn't cause such marked hypoxia.
