Cyprian needs to have an ELIZA sent to confirm the diagnosis.

  • She needs to be referred to 'Clinic 49' for follow-up of HIV/AIDS.
  • Why the pancytopenea?
  • HIV itself
  • Infiltration of marrow with M. tuberculosis or atypical mycobacterium.
Her treatment consisted of
  • Septrin and prednisone for her PCP  --->
  • Oral fluconazole for her thrush.
  • Nutritional supplements.
  • 'Gentle' hydration.

A CD4 count is simply too expensive to order routinely.

The differential diagnosis for her pulmonary symptoms should include:

  • Miliary tuberculosis
  • Malignancy with lymphangitic spread.
  • Pulmonary embolus
  • Atypical pneumonia should be kept in mind, but generally doesn't cause such marked hypoxia.

Grace is our next patient. She has a productive cough and 'hotness of the body'.
